Your Opportunity:

Connecting People & Community for Living Well (CPCLW) initiative within Assisted Living Alberta is looking to hire two new provincial team members. The initiative has deep roots in rural Alberta and will be expanding to new communities. The initiative works with health, social and community partners to build and sustain community led projects that enhance the wellbeing of local underserved populations. We are looking for passionate individuals experienced in strengthening aging in place and collaboration using a community development approach. Using the CPCLW Model, Wellbeing Guide, and identified learning areas, the Practice Lead (PL) will guide rural community teams to identify local priorities, plan for the implementation of local programs and supports, and measure and monitor the impact of supporting wellbeing locally. This is a provincial out of scope position within Assisted Living Alberta and is supported by a Health Canada grant until Mar 2029.

Description:

The PL will support: Spreading the model to new rural communities looking to impact the wellbeing of those living with dementia, caregivers and other underserved groups. Sustaining the model in participating communities. Sharing learnings from the work. Building a knowledge base to inform policy and infrastructure planning. Organizing education opportunities to address the learning needs. Evaluating implementation of the model. Completing Social Return on Investment (SROI) analysis. The PL will provide expertise and leadership related to the CPCLW initiative including: Contributing to the deliverables and impact of the CPCLW team and local communities. Supporting existing new community-based teams to implement the CPCLW model and local action planning. Connecting with appropriate sectors, partners, communities and team members to help local team’s progress and managing key project work activities. Working with partners across the province to enable the development of innovative solutions during the planning, implementation, evaluation, and sustainability. Implementing community development strategies for supporting inter-professional collaboration among multi-sector community teams, members of the public, those with lived experience, and is a role model for effective collaboration. Thriving in a complex system and changing environment, managing complex issues and supporting shared leadership. Applying appropriate knowledge, skills, tools and techniques to contribute to the development, implementation and evaluation of key project activities.

Required Qualifications:

Undergraduate degree in public health, health promotion, social work, community development, gerontology, nursing, or related social/health science; equivalent combinations of education and lived/professional experience considered.


Additional Required Qualifications:

3–5+ years leading/facilitating community action and program delivery in health/public health, seniors’ services, home care, community development, or related sectors (multi sector collaboration, service design, advisory structures, stakeholder relations). Facilitation and collaboration skills with community /diverse partners for multistakeholder settings; partnering with those with lived experience and diverse populations; support team decision making and problem solving. Health system integration (e.g., seniors health & continuing care, primary health care integration) experience, moving evidence into practice within a complex system, the Alberta health and social care system, and rural environment an asset. Trust based relationship building across communities and teams; wellbeing and strengths-based approaches; inclusive, culturally safe engagement; collaboration with project team, leadership team and other internal and external leaders and stakeholders to effectively contribute to grant related activities. This position will require travel within Alberta.


Preferred Qualifications:

Master’s degree preferred. Graduate training (MPH, MSW, MN, MEd/Health Promotion, MSc/Health Services). Current registration with a health professional association preferred. Complex project leadership experience to guide teams through undefined, evolving plans with multiple stakeholders, inspires and builds confidence; ongoing exploration of new approaches and perspectives, project management, activating change, knowledge translation and quality improvement practices.
