Your Opportunity:
The Social Worker II in the South Health Campus Rapid Access Unit (RAU) supports the unit’s mandate to provide short‑term, goal‑focused assessment and treatment for patients who require rapid stabilization and timely discharge. The RAU model of care emphasizes early interdisciplinary assessment, proactive discharge planning, and efficient transitions to community, home care, or continuing care services. Working within a condensed length of stay, the Social Worker II provides focused psychosocial assessment, brief intervention, and discharge support at the front end of the admission. This role is integral to identifying psychosocial risks that may impact discharge readiness and addressing barriers related to housing, caregiver capacity, functional change, safety, or access to supports. The Social Worker II collaborates closely with physicians, nursing, allied health, and community partners during daily rapid rounds and care planning discussions. Social work input supports criteria‑led discharge decisions, reduces unnecessary inpatient admission, and promotes care in the most appropriate setting. This position is well suited to a social worker who thrives in time‑limited intervention, values measurable outcomes, and brings strong system navigation skills to support patient flow and safe transitions consistent with Alberta’s health system priorities.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum two (2) years of post‑degree social work experience, preferably in acute care, short‑stay units, or complex community‑based settings. Demonstrated ability to complete efficient, clinically focused psychosocial assessments within tight timeframes. Experience providing brief, goal‑directed interventions aligned with rapid discharge planning. Strong understanding of Alberta community resources, including Home Care, income supports, housing services, and continuing care pathways. Ability to function effectively in a high‑turnover environment where prioritization and rapid decision‑making are required. Demonstrated ability to contribute meaningfully to interdisciplinary rapid rounds and care planning discussions. Strong communication and concise clinical documentation skills. Proficiency with electronic health records and clinical information systems (Connect Care/EPIC preferred). Ability to work independently within defined Social Worker II scope and practice standards.
Preferred Qualifications:Experience working in a Rapid Access Unit, short‑stay medical unit, or acute care setting. Experience supporting older adults or patients with functional or cognitive change impacting discharge readiness. Familiarity with criteria‑led discharge models and transition‑focused care. Knowledge of trauma‑informed and culturally responsive practice.
