Your Opportunity:

The Social Worker II in the South Health Campus Emergency Department plays an essential role in supporting Alberta’s provincial priorities for Assisted Living Alberta and improved transitions of care. Social work intervention in the Emergency Department is critical to addressing the psychosocial complexity that impacts patient flow, discharge readiness, and avoidable hospital use. Operating within a fast‑paced, high‑acuity environment, the Social Worker II provides timely psychosocial assessment, crisis intervention, brief therapeutic support, and discharge planning for patients presenting with complex social, family, housing, cognitive, or safety concerns. This role focuses on mitigating immediate psychosocial risk and supporting safe, appropriate transitions from the ED to community, home care, or assisted living settings. The Social Worker II practices with a high degree of independence within established guidelines and works collaboratively with physicians, nursing, allied health, and community partners. Particular focus is placed on supporting older adults and individuals whose social circumstances, functional change, or caregiver stress may benefit from Assisted Living Alberta pathways or other continuing care supports. This position contributes directly to patient safety, team decision‑making, and system flow, supporting Alberta Health Services’ commitment to delivering the right care, in the right place, at the right time.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum two (2) years of post‑degree social work experience, preferably within health care, acute care, or complex community‑based environments. Demonstrated competence in psychosocial assessment, crisis intervention, and brief, evidence‑informed therapeutic approaches. Ability to practice effectively in a high‑volume, time‑sensitive environment requiring rapid prioritization and sound professional judgment. Strong working knowledge of Alberta community resources, social support systems, and continuing care pathways. Demonstrated ability to collaborate effectively within interdisciplinary health care teams, contributing social work expertise to shared clinical decision‑making. Experience supporting patients and families through discharge planning and care transitions, including addressing barriers related to housing, caregiver support, safety, and access to services. Strong verbal and written communication skills, including concise clinical documentation. Proficiency using electronic health records and clinical information systems (Connect Care/EPIC preferred). Ability to work independently within defined role scope while recognizing when consultation is required.

Preferred Qualifications:

Experience in an Emergency Department or acute care setting. Experience supporting older adults, individuals with cognitive impairment, or complex discharge needs. Familiarity with Assisted Living Alberta, Home Care, and Continuing Care Access processes. Knowledge of trauma‑informed and culturally responsive practice.
