Social Worker II
Your Opportunity:
Working collaboratively and as part of an interdisciplinary team, the Social Worker II provides bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive counselling, advocacy, and consultation as part of their day-to-day work. The Social Worker II has excellent working knowledge of community and financial resources (AISH, Income Support, Canada Pension, Alberta Seniors Benefits) as well as various legislative acts that impact health services (Personal Directives Act, AGTA, Mental Health Act). The Social Worker makes appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of patients and families. This position requires the ability to prioritize workload, analyze complex problems and identify viable care plans and solutions to address psychosocial needs. The incumbent demonstrates the ability to work both independently and as a member of an interdisciplinary team. The Social Worker II contributes to non-patient care activities including program planning, evaluation, quality improvement research and staff development, along with other duties as required by the position. This position’s responsibility is to cover the designated medical/surgical units in addition to providing additional coverage on other medical units as needed.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 2 years of professional Social Work experience. This position requires a high degree of independence, critical thinking, problem-solving skills, and an ability to prioritize workload. Knowledge and ability to use computer applications such as Microsoft Office Suite, Meditech and/or Connect Care. Previous experience working within a health care system.
Preferred Qualifications:Excellent verbal and written communication skills. Experience working in an interdisciplinary team is preferred. Knowledge of relevant legislative Acts including Personal Directives Act, AGTA and awareness of capacity assessment processes is an asset. In-depth knowledge and in working with Income Support programs at a community/ provincial/federal level (AISH, Alberta Works, CPP, OAS, ASB, EI, etc.) is preferred.
