Social Worker II
Your Opportunity:
Working collaboratively and as part of an interdisciplinary team, the Social Worker II is involved in the provision of services to patients and families at Medicine Hat Regional Hospital; largely on the inpatient psychiatry unit (5N) but also supports other floors in the hospital and in the community as required. The Social Worker II provides bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive counselling, advocacy and consultation as part of their day-to-day work. The Social Worker II has excellent working knowledge of community financial resources for adults and their caregivers. The Social Worker makes appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of patients and families. This position requires the ability to prioritize workload, analyze complex problems and identify viable care plans and solutions to address psychosocial needs. The incumbent demonstrates the ability to work both independently and as a member of an interdisciplinary team. The Social Worker II contributes to non-patient care activities including program planning, evaluation, quality improvement research and staff development, along with other duties as required by the position.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 2 years of professional Social Work experience. This position requires a high degree of independence, critical thinking, and problem-solving skills, an ability to prioritize workload. Knowledge of relevant legislative Acts and experience with guardianship, trusteeship, personal directives and capacity assessment processes is an asset Knowledge and ability to use computer applications such as Microsoft Office, Outlook and Teams. Previous experience working in the health care system and EMR such as Connect Care. Excellent written and verbal communication skills.
Preferred Qualifications:Experience working in an interdisciplinary team. Experience working with Income Support programs at a community/provincial/federal level (AISH, Alberta Works, CPP, OAS, ASB) is preferred.
