Social Worker III - Clinical Practice lead

The Royal Alexandra Hospital is a fast-paced exciting facility that provides opportunities to work, learn and grow within an interdisciplinary team. The Social Worker III, Clinical Practice Lead, will provide clinical leadership and mentorship to Social Workers in an acute care hospital setting and support the social work staff to ensure safe, efficient, evidence-based, patient and family-centered, inpatient and outpatient care. Responsibilities include the development and implementation of Social Work practice and documentation standards, supervising the clinical performance of social workers and developing consistent practices and processes. This position will provide leadership internally and develop relationships with internal and external partners to assist with patient flow in and out of hospital. The incumbent will provide direct patient care on an as needed basis. Social Work activities may include psychosocial assessment, crisis intervention, complex discharge planning, financial assessment, capacity assessment processes, education, treatment, advocacy, resource finding, consultation, making appropriate referrals both internal and external to the hospital and liaising with community agencies/programs. This position will work closely with the Social Work manager and the Social Work team leaders. Description:
As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

Master's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Graduation from a recognized university Social Work program with a Master of Social Work Degree is required. Registration with the Alberta College of Social Workers is required. A minimum of 3 years clinical experience in a large urban acute care setting is required. Excellent critical thinking, communication and organizational skills are a must. A proven ability to provide clinical leadership, teach and mentor is critical to being successful in this position. Knowledge and experience with complex discharge planning and decision making is an asset. Experience with Connect care electronic medical records is an asset.
