Your Opportunity:

Working collaboratively as a member of the Palliative Care Team based out of Medicine Hat, the Social Worker III is involved in providing compassionate advanced level bio-psychosocial assessment and client centered interventions with clients and families as the progress along their palliative journey. As Social Worker III working with your interdisciplinary team, you will provide a broad range of advanced psychosocial supports. These may include counselling supports related to end of life, bereavement and complex grief, and addressing client and family care needs for conflict resolution, risk assessment, advocacy, and consultation. The Social Worker III will have expert knowledge of financial benefits programs and community-based resources and will provide resource navigation, creative problem solving, and appropriate referral and consultation as required. You will provide advanced consultation and clinical direction pertaining to the application of relevant legislative acts that impact direct patient care (including Adult Guardianship and Trusteeship Act, Personal Directives Act, Mental Health Act, Medical Assistance in Dying). Services may be provided to individuals, families and groups. Travel throughout South Zone is required.

Description:

As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

Required Qualifications:

Master's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Previous experience with Palliative approach to care and provision of counselling in the areas of bereavement, complex grief and loss.

Preferred Qualifications:

MSW with a minimum of 3 years' experience in a medical/healthcare setting.
