Your Opportunity:

Reporting to the Area Manager for Transition Services (TS) Community this advanced level Social Work clinical practice leader (CPL) will support all TS community programs including the Intensive Case Management teams in acute care to advance clinical practice and provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. The Social Worker III position involves provisions of advanced clinical services, consultation to the inter-professional team, patients/families and supervision of students and team Social Workers. This role provides mentoring and support to all inter professional team members involved in the care planning for Transition Services complex client population. The SW CPL is recognized as an expert clinical practitioner, providing leadership and consultation in complex clinical cases including those of internal and external stakeholders/partners. The SW CPL demonstrates expertise in facilitation of program development, service planning, staff development and quality improvement and evaluation. This position will engage and working collaboratively with several internal and external stakeholders to develop processes to promote enhanced resource utilization while assisting with smooth client transitions and optimizing client outcomes. The CPL achieves advancement of SW clinical practice through the development of and ensuring implementation of clinical policies, procedures and standards and scholarly contributions to Social Work professional standards and governance. The CPL functions as a leader, mentor and change agent.


As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as clinical lead providing leadership in the planning, development, delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

Required Qualifications:

Completion of master's degree in Social Work. (equivalent education or experience may be considered). Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum 5 years clinical experience with 3 years in community focused practice setting. This is multisite position requiring a vehicle and ability to be at multiple locations throughout the day when necessary.

Preferred Qualifications:

Seniors and Mental Health experience an asset. Gerontology certifications an asset. Continuing care experience an asset. Extensive knowledge of the patient population and leading practices related to Transition Services, community and government programs with in Calgary zone and Alberta. Team leadership skills including facilitation, coaching, mentorship and teaching.
