As one of our Social Worker team you will provide generalist services in close collaboration with acute care and community organizations in Continuing Care (Type A and B) facilities. This position provides a range of Social Work services to patients and their families where they are living. These services are primarily clinical in nature and require expertise in financial and resource management, psychosocial assessments, counseling, case management and discharge planning. This also involves consultation and collaboration with medical staff and community agencies. This position requires the ability to work independently with occasional review, consultation and evaluation from a Manager or Coordinator and the ability to work as part of an inter-disciplinary team. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Completion of a recognized Bachelor of Social Work degree required. Registration with the Alberta College of Social Workers (ACSW) required. Current BCLS-HCP certification required. Minimum of two years’ experience in clinical area of medical social work required, and specific experience in the home care environment is essential. Excellent communication skills, written and verbal. Ability to establish positive working relationships with a wide variety of patients and families. Ability to organize and set priorities. Considerable initiative and judgment required. Experience working on an inter-disciplinary team required. Good computer skills are required (Connect Care training is preferred). Knowledge of and experience with accessing and navigating a variety of community resources to support in the management of clients with complex needs.
