Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. The RSW-SW II works independently to provide a range of generalist social work interventions and to manage a complex caseload of Continuing Care clients across the Edmonton Zone. The focus of the position is to resolve outstanding social, legal and/or financial barriers for clients transitioning to and residing in Continuing Care centers. The RSW-SW II collaborates with inter-professional health team members, public sector, and community agencies to advance care for clients and participates in quality improvement initiatives. Key responsibilities will include and are not limited to: Assesses for eligibility to social programs, benefits and resources. Counsels, advises and supports clients in arranging available resources to support transition to a CCH. Completes risk screening and assessment and timely care transition/discharge plans. Case management involving the coordination of providers, services and resources to enhance coordination and integration of services and care for clients and families. Facilitates problem solving process with clients and families with decision-making issues. Develops collaborative partnerships to facilitate access to services. Applies knowledge of relevant legislation to assists members of the health care team in delivering services within legislative requirements (e.g. Adult Guardianship & Trustee Act (AGTA), Personal Directives Act). Contributes to cognitive testing/capacity assessment.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). The candidate will have proven communication skills to work cooperatively and effectively with the healthcare team members, patients and their families in a fast-paced environment. Strong professional judgment, initiative, and effective time management skills. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. Prior experience with the Bridge to Income Support program is required.
Preferred Qualifications:Experience in a Continuing Care setting. Knowledge of Meditech and Connect Care. Palliative Care Experience.
