The Northern Alberta Cardiac Rehabilitation Program is an exercise and education program geared towards providing participants with the knowledge, skills, and supports they need to better manage their health after a heart event. Working to achieve the vision of the Northern Alberta Cardiac Rehabilitation Program, care is focused on promoting exercise and healthy lifestyle management. The Social Worker II will play an important role in supporting patients through their cardiac rehabilitation experience. Reporting to the Assisted Living Social Work Manager, The Social Worker Il provides a range of Social Work services to patients and their families within the established standards, directives, and policies for the work environment. These services may include psychosocial assessment, resource-based interventions, risk screening, counselling, transition planning, prevention & promotion, and public awareness activities. There is a large focus on providing individual and group patient education and supportive counselling. The successful candidate must be able to work independently with a high degree of judgment and must be able to work effectively as part of an interprofessional team.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 1 year of experience working at the Glenrose Hospital with older adult patients. Strong knowledge of various financial/medication coverage resources such as disability benefits, EI, AISH, etc. Able to function as a consultant and educator. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Proficient computer skills including Microsoft Office and Connect Care. Knowledge and understanding of Adult Learning Principles and Motivational Interviewing. Demonstrated ability to work with a multi-disciplinary team and practice in a patient/family centered environment. Effective clinical reasoning/problem solving skills, analytical thinking and conflict resolution. Demonstrates initiative, adaptability, flexibility, leadership, and organizational skills to meet clinical and operational needs as required.
