Our state-of-the-art facility provides specialized treatment services to patients in the Edmonton and Northern Alberta area. The SW II will thrive within a supportive, interdisciplinary team that is dedicated to ensuring safe, efficient, evidence-based, patient and family-centered, inpatient and outpatient care. The SWII is responsible for the assessment and treatment of patients as well as development, implementation, and evaluation of treatment programs according to professional practice standards, program development and evaluation, student supervision and staff education. Our team of passionate health care professionals are dedicated to enhancing the quality of life of those affected by illness or injury by providing comprehensive support and care from the time of diagnosis, throughout treatment and discharge back into community. Specifically, you will join a progressive team of Allied Health Professionals that include SW, SW assistant, Housing Support worker, Transition Coordinators, OT’s, PT’s and more that continuously collaborate to provide the best care for patients and have opportunities for ongoing mentorship.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Recent work experience as a Social Worker (within the past three years) in a hospital setting, preferably on a medicine or surgery unit. Strong skills in crisis intervention, psychosocial assessment, and complex discharge planning. Demonstrated ability to make timely decisions and prioritize effectively in a fast-paced environment. Excellent verbal and written communication skills, including proficiency in clinical documentation. Ability to work both independently and collaboratively within an interdisciplinary team. Knowledge of Edmonton-area community resources and referral pathways. Experience working with individuals facing houselessness, poverty, substance use, and mental health challenges. Demonstrated culturally safe and trauma-informed practice.
Preferred Qualifications:Minimum of one (1) year of recent acute care experience, working in a hospital environment, on a medicine or surgery unit. Strong working knowledge of hospital electronic medical records systems (e.g., Connect Care).
