This job opportunity is for a temporary full-time Social Worker II in the Vegreville/Lamont area. This provides professional social work services in clients’ homes, continuing care facilities, and hospital settings. As part of a multidisciplinary team, you will: Conduct psychosocial assessments, identify unmet needs and develop care plans, provide social navigation and resource-based interventions, collaborate with clients, families, and decision-makers to achieve client goals.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum of one (1) year of recent experience (within the past three years) in an acute-care hospital setting within an urban environment. Relevant Social Work experience in fast-paced, crisis-response, or high-demand service settings will also be considered. Demonstrated skills in crisis intervention, psychosocial assessment, case management, and complex discharge and transition planning. Proven ability to prioritize, take initiative, and make sound clinical decisions in a rapidly changing environment. Strong verbal and written communication skills, including proficiency in clinical documentation. Ability to work both independently and collaboratively within an interdisciplinary team. Experience working with individuals facing houselessness, poverty, substance use, mental health challenges, and complex social circumstances. Commitment to culturally safe and trauma-informed practice, with experience applying harm reduction approaches when supporting individuals with intersecting psychosocial and health-related needs.
Preferred Qualifications:Experience working as a Social Worker in an Emergency Department within an urban setting. Knowledge of community resources, referral pathways, and interagency collaboration.
