This position offers an exceptional opportunity to practice to your full scope as the Social Worker for the Intensive Care Unit at Red Deer Regional Hospital. Working in a fast-paced acute care environment, you will collaborate within an interdisciplinary team to serve a diverse population of critically ill patients and their families. Key responsibilities include conducting comprehensive psychosocial assessments, developing and implementing intervention plans, and providing trauma informed care, adjustment counseling, grief support, crisis intervention, and discharge coordination. You will also make referrals for ongoing treatment and actively participate in discharge planning. This role requires strong knowledge of relevant legislation, including the Personal Directives Act, Adult Guardianship and Trusteeship Act, and Child, Youth and Family Enhancement Act. A solid understanding of community resources and financial supports is essential, along with effective time management skills. Strong written and verbal communication abilities are critical to success in this position. Additionally, the Social Worker will build and maintain collaborative partnerships with community service providers to ensure safe, patient centered discharge plans.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Strong communication and collaboration skills, ability to work independently in diverse settings (homes, congregate living, facilities, hospital) professional judgment, initiative, and time management, proficiency in Microsoft Office. Knowledge of Guardianship and Trusteeship Act, Personal Directive Act, Enduring Power of Attorney Act.

Preferred Qualifications:

Familiarity with Connect Care. Palliative Care experience. Opportunities for professional growth and development.
