Come join the Calgary Zone rural social work team at Oilfields Hospital in Black Diamond! Reporting to and working alongside the Acute Manager and Unit Manager, Oilfield’s Hospital social worker functions as a key point of contact to assist patients and families in the Emergency Department, Medical Inpatient Unit and Long Term Care Facility. Using AHS core values and a patient family centered care model the social worker for Oilfields Hospital will lead transition service support, complex discharge planning and proactive client management. This position works with an inter-disciplinary team and the successful candidate must have excellent verbal and written communication skills, adaptive personality, along with the ability to think critically.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 2-5 years experience working in a hospital setting or comparable experience in a related field. Time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic, interdisciplinary environment. Self-directed and independent decision making, sound clinical judgment and critical thinking skills. Excellent communication and interpersonal skills. Embraces teamwork.
Preferred Qualifications:Creative, innovative, solutions focused to manage complexity. Team player who can also work independently. Experience with program development. Experience working in a rural setting.
