Social Worker II
Your Opportunity:
The Transition Services Coordinator Social Worker (TSC SW) in the Alternative Level of Care (ALC) Program is a part of the multidisciplinary team that provides person centered care for individuals who require facility supported transitional care or an extended period of convalescence with a view of returning home. The TSC SW provides a wide range of services for clients across the health care continuum. The TSC SW has experience in the role of advocate and can balance the needs of the individual/family/community with organizational and legislative parameters, including financial accountability for allocation of Alberta Health Services funding to clients. Clients are moved from acute care sites in the Calgary Zone to ALC units where the TSC SW conducts ongoing assessment and discharge planning. Clients may be assessed for Long Term Care (LTC), Supported Living (SL) or discharged home with home care supports. The TSC SW will also be required to assess clients for the most appropriate designated living option using the RAI Assessment and create pathways profiles. You will attend discharge planning rounds on the unit and support transitions in care with clients and families. The ALC program is based at Carewest Sarcee 3504 Sarcee Rd SW. This position is a sedentary position and requires long periods of keyboarding and computer use. May be keyboarding up to 6 hour a day. Due to the nature of the work, long periods of time will be spent in a seated position.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Two years of recent experience preferably in a Community Health and Acute Care settings Experience working with geriatric, mental health or complex care adult populations Previous experience working with an interdisciplinary team.
Preferred Qualifications:5+ years experience in Mental Health, Home Care, Geriatrics or Transition Services. Community Experience (Home Care or Transition Services). SCM, Connect Care and Pathways experience
