Your Opportunity:

The Social Worker II Assisted Living Alberta plays an essential role in supporting patient transitions and coordinating care across Acute Care, Home and Community Care, and Continuing Care Home settings. This position delivers comprehensive clinical social work services with a strong emphasis on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. In this role, you will assist patients and families in accessing financial benefits, completing funding applications, and liaising with government programs, community agencies, and other service partners to ensure appropriate supports are in place. You will work collaboratively with an interdisciplinary team—including physicians, nursing staff, allied health professionals, and external providers—to develop and implement holistic care plans that promote safe, effective, and sustainable transitions. The ideal candidate demonstrates strong clinical judgment, exceptional communication skills, and the ability to work both independently and within a collaborative healthcare environment. Success in this role requires adaptability, critical thinking, and a commitment to delivering patient centered care in a dynamic and fast paced setting. This position primarily supports the communities of Slave Lake and Wabasca, with the possibility of providing services to nearby communities as needed. Responsibilities span Acute Care, Home and Community Care, and Continuing Care Homes. Travel is required, and a reliable vehicle with business insurance is necessary. Mileage or travel-related compensation is provided by the employer. In addition to a competitive rate of pay, Assisted Living Alberta currently offers a Remote Retention Allowance of up to $3,000 annually. This allowance is non pensionable and paid hourly for all hours worked at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Certification in Basic Life Support - Healthcare Provider. Drivers License with vehicle suitable for North Zone travel 12 months of the year.

Preferred Qualifications:

Connect Care training. Non Violent Crisis Intervention certification. Designation as a Capacity Assessor. A minimum of one year of experience in Acute Care, Continuing Care, and/or Home Care. Enhanced knowledge of community based supports and services. Experience and knowledge related to AHS/ALA system navigation, care placement processes, transitions in care, and available financial supports.
