Social Worker II
Your Opportunity:
This Social Worker II position with the Provincial Virtual Rehabilitation intake team is a frontline, patient-facing clinical role. The intake team supports multiple programs, but primarily supports the Provincial Prehabilitation Program. Prehabilitation (Prehab) helps patients to prepare their mind and body for surgery. The AHS Prehabilitation Program offers service options across four domains – Feeling Ready, Being Active, Eating Well, and Living Well. The team offers universal, targeted, and personalized services for pre-surgical patients across Alberta. As the Social Worker II, your role will be focused on Intake (referral management, chart review, clinical triage, wayfinding, clinical recommendations, Social Work interventions), with daily support and supervision by the Intake Team Lead. Your daily work will include calling patients, providing patient appointments by phone and zoom and supporting patients to access appropriate service options. While your role will primarily focus on patients entering the program, you will also be involved in follow-up, and will collaborate across the team, contributing to the delivery of universal and targeted prehabilitation interventions. We are looking for an enthusiastic clinician who can efficiently manage large patient volumes, identify process and system-level improvement opportunities, build partnerships with clinicians and leaders, and establish Prehab services that impact patient and healthcare system outcomes. The AHS Prehabilitation Program is relatively new, with an ongoing focus on provincial implementation and evaluation. This SW II position requires strengths in program development, innovative practice, flexible thinking, and in negotiating change as we continue to grow as a team and program. This position requires the ability to manage a busy caseload with significant volumes.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum of 6 years of experience in healthcare, including community rehab, acute care, homecare or surgical service settings. Experience with and knowledge of prehabilitation. Knowledge of patient population needs, health care continuum, and leading practices in collaborative, interdisciplinary health services and rehabilitation. Strengths in clinical triage, wayfinding, supporting patients and families. Excellent oral and written communication skills. Strong interpersonal skills, including an ability to develop therapeutic relationships and navigate sensitive situations. Ability to create effective internal and external partnerships. Aptitude for performing in an environment of innovation, continuous change, and learning. Excellent clinical reasoning and decision-making skills. Expert presentation and written acumen skills. Expert technical and virtual service delivery skills. Initiative and ability to work independently within a remote team. Exceptional technical skills in managing computers and software, including Microsoft Office Suite and other e-platforms and web-based tools.
Preferred Qualifications:Connect Care experience with data reporting is an asset. Technical aptitude is an asset. Experience working as a triage clinician, referral management or case manager.
