Your Opportunity:

The job opportunity available is for a temporary part time social worker II position, with the potential to work full time hours for 6 months. The successful incumbent will provide services and work as part of an interdisciplinary team in the following service streams in Two Hills: SAGE, Acute Care, Home Care, and Long-Term Care.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
