Your Opportunity:

This part-time temporary Social Worker II position in Community Care provides services through Home Care to clients in their homes and supportive living facilities. The social worker will provide resources and supports to clients and families along their home care journey. Awareness of resources including housing, finances, counselling addiction and mental health is essential in this role. Previous work in a hospital or long-term care setting would benefit this position. This position is working with seniors and individuals with physical and mental disabilities, as well as the extended interdisciplinary team. Travel within the zone is required.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

BLS/First Aid current. Minimum 3 years of experience as a professional social worker is required. Effective communication and interpersonal skills are required. Please note: a minimum of BSW and ability to register with ACSW is required, equivalencies will not be considered.

Preferred Qualifications:

Previous hospital or home care experience is preferred. Broad knowledge of local community resources is essential. Ability to demonstrate effective time management and prioritization skills is preferred. Experience with Connect Care/EPIC.
