Your Opportunity:

This Social Worker II is a Rural Social Work position working across the continuum of Acute Care, Home Care, and Facility Living. You will work as part of an Interdisciplinary team, assessing, intervening and contributing to the overall plan of care of clients across the life spectrum. A broad knowledge of local, provincial and federal resources is required to help facilitate wellness for clients accessing health services. Travel within the Zone is required, and this position works out of the Raymond Health Centre.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Valid Driver's license and dependable vehicle required.

Preferred Qualifications:

Minimum three years’ experience as a social worker. Experience working in a rural health care setting preferred. Previous experience working with individuals along the placement process is preferred.
