Your Opportunity:
The Social Worker Il provides a range of social work services to patients and their families on an inpatient mental health unit in an acute care setting. Working within the context of family- centered care and multidisciplinary service delivery, the successful applicant will collaborate with a range of other service providers within AHS and community to provide support and clinical intervention to patient on an in-patient mental health unit. The focus of this position will be with discharge planning to ensure seamless patient care and transition back into the community upon discharge. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Basic Cardiac Support- Health Care Provider (BCLS-HCP) certification required. Demonstrated ability to collaborate with other service providers and to adapt practice to the unique needs of a range of patients, their families and community service providers required. Excellent knowledge and understanding of the Assured Income for the Severely Handicapped (AISH) program. Employment Insurance (El) program, residential treatment facilities, social service and how to assist patients in accessing these programs as required. Strong mental health assessment and crisis management skills required. Solid knowledge of intervention and treatment modalities relevant to clinical disorders, psychosocial and environmental problems required. Comprehensive knowledge of relevant community resources and legislation required. Experience in discharge planning for patients transitioning back into the community required. Minimum 2 years Social Worker experience providing clinical services to Addictions and Mental Health clients in acute psychiatric inpatient unit. Connect Care experience.
Preferred Qualifications:NVCI. Master's degree preferred.
