Your Opportunity:

The Social Worker II - Family Health provides services at the Chinook Regional Hospital in the Family Health care areas. Services include providing support, resources and advocacy from pregnancy to working with children and parents on their health journey. Awareness of community resources including housing, finances, addiction and mental health, crisis, grief, and domestic violence is essential in this role. Previous health care experience would benefit this role. The role involves working with children and families, an interdisciplinary team, and community partners.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

BLS/First Aid is required. Minimum 3 years previous social work professional experience is required. Effective communication and prioritization skills is essential.

Preferred Qualifications:

Previous hospital professional experience is preferred.
