Your Opportunity:

Reporting to Therapist Specialist, the Social Worker II performs a wide range of social work duties including interventions, counselling, support services, discharge and after-care plans to promote the health and wellbeing of clients experiencing social, financial and/or health concerns. Key Responsibilities: Conducts psychosocial assessments, identifies client issues, formulates and carries out appropriate social work treatment plans in order to meet client needs. Provides counselling interventions to clients and families faced with multi-faceted problems. Facilitates and coordinates individual, group and/or family counselling or conferences, often involving interdisciplinary teams. Provides crisis counselling, mediation and conflict resolution strategies. Advocates for clients by coordinating the efforts of all internal and external parties to meet client and family psychosocial and instrumental needs such as housing, food, transportation, income support and financial management. Supports clients in completing applications and navigating complex systems. Develops and follows up with discharge and aftercare plans. Supervises bachelor and/or diploma level social work practicum students. Promotes relationships and effective liaison with agencies and stakeholders, and in some cases, promotes community awareness of social work or health issues. Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Registration with the Alberta College of Social Workers (ACSW). 5-7 years’ experience working with at risk families providing counselling interventions and support around intrinsic needs. Minimum 1 year experience working in a primary health care setting. Demonstrated ability to work with individuals from diverse backgrounds and cultures using a supportive, non-judgmental approach.

Preferred Qualifications:

Experience with adult education and group facilitation. Demonstrated ability to collaborate effectively with various community partners required. Demonstrated ability to function in a collaborative interdisciplinary environment required. Demonstrated flexibility and adaptability within a complex and rapidly changing environment. May involve working with clients who present criminal or suicidal tendencies with potential exposure to violence and aggression.
