Your Opportunity:

The Halvar Johnson Centre for Brain Injury is a 48-bed inpatient rehabilitation program. It is a high intensity program designed for people with a moderate to severe non-progressive acquired brain injury between the ages of 16-65. Reporting to the unit manager, the role of the Social Worker II is to work collaboratively within an interdisciplinary team to provide excellent Patient and Family Centered Care utilizing the core values of AHS. Your role will be to facilitate comprehensive psychosocial assessment, assist the clients and families with achieving their recovery goals, provide patient and family support and education, lead family meetings, provide client/family advocacy as needed and engage clients in supportive counselling as required. You will work closely with the patient, family/supports, treatment team and community partners to identify and secure appropriate community resources and supports to ensure a successful discharge. This position has transitioned to Recovery Alberta effective September 1, 2024. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

You have excellent observation, communication and organization skills and a demonstrated ability to work collaboratively in an interdisciplinary team. You are able to communicate findings in a concise and accurate manner. You have strong problem solving and interpersonal skills. Self-direction and independent decision making, sound clinical judgment and critical thinking skills. Time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic and challenging environment. Effective communication with a patient and family centered focus. Demonstrates problem solving, critical thinking and conflict resolution skills. CPR required – BLS for Health Care Providers Level C. You are physically capable of carrying out the duties of this position as you may be required to transfer, reposition and mobilize patients as well as manage behavioral problems resulting from the brain injury.

Preferred Qualifications:

Greater than six months experience in working in neurology, with stroke and brain injured patients is desired.
