Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Knowledge and experience with varied community resources. Skills in case management, crisis intervention, safety planning, system navigation and advocacy. Strong assessment, time management and prioritization skills. Knowledge of provincial and federal legislation, social work ethics and practice standards impacting the role and patient population. Proficiency using computers and Connect Care, Netcare, Microsoft Office. Maintains accurate and timely records. Utilizes evidence-based practice standards and critical thinking to generate effective solutions. Strong written and verbal communication skills. Ability to adapt style to meet patient needs while setting appropriate boundaries within the therapeutic relationship. Uses resources effectively and efficiently. Demonstrates focus and resilience in challenging situations. Maintains professionalism with patients and within MDT.
Preferred Qualifications:Standard Cardiopulmonary Resuscitation (CPR) Certification at the Provider Level C. Community based experience with vulnerable populations and individuals with complex high-level needs. Ability to perform restricted psychosocial interventions is an asset, although not required.
