Social Worker II
Your Opportunity:
The social work position reports to the Care Manager of the Rural South Team in the Integrated Home Care program and provides a range of social work services to clients, families and an interdisciplinary team. The IHC program supports clients to remain independent in the home. Assess client functioning, financial position, illness adjustment, coping, informal supports, housing issues, addictions, interpersonal violence, family dynamics, cultural diversity issues and incorporate it as a part of the care plan. Provide support, collaboration and advocacy to both client and their friends/family members along with other disciplines or agencies. Facilitation of capacity issues, Advance Care Planning and transition to more supportive living environments. Maintain electronic records. Provide crisis intervention by liaising with emergency services, as needed.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Excellent communication skills, written and verbal. Considerable initiative and judgement and the ability to work independently in community settings is essential. Basic computer skills.
Preferred Qualifications:Knowledge of and experience with guardianship, trusteeship and capacity assessment processes is an asset. In-depth knowledge and experience in working with Income Security programs at community/provincial/federal levels (Alberta Works, AISH, CPP) is preferred. Minimum 2 years social work experience in a health care setting. Experience in community work is preferred.
