Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Current BCLS-HCP certification is required. Proficiency in computer applications including word processing, email, internet use, and electronic charting is essential. Strong assessment, communication, organizational, problem-solving, counseling, and group facilitation skills are required. Must work independently with sound judgment and collaborate effectively within an interdisciplinary team. A strong commitment to patient- and family-centered care is essential, along with the ability to foster partnerships that enhance outcomes. The role involves extended periods of sitting and cognitive load while delivering virtual care via Zoom and Connect Care. Physical demands may include transporting equipment (20–50 lbs) and bending/rotation during clinical duties. Recent experience in case management or service coordination for clients with diverse rehab needs is required. Experience working with patients with complex rehabilitation needs, knowledge of community and support services, and recent interdisciplinary team experience are all required. Experience with Connect Care scheduling and EMR, and training in HealthChange Methodology™ are also required.
Preferred Qualifications:Experience with diverse cultural groups and new immigrants, applying cultural humility and adapting care to varied needs. Skilled in supporting care transitions across settings. Knowledge of trauma-informed care and harm reduction. Experienced in cross discipline mentoring, quality improvement, and research.
