Your Opportunity:

Reporting to the unit manager, the Social Worker II (SWII) works as a member of a health care team, collaborating on the delivery of care to meet the needs of patients. The SWII is responsible and accountable to the public for his/her own practice, according to the standards defined by the Alberta College of Social Workers (ACSW). The SW II will provide a range of social work services to patients and their families in an acute care setting. Working within the context of patient and family-centered care and multidisciplinary service delivery, the SWII will collaborate with a range of other service providers within AHS and community partners to provide support and clinical intervention to patients on the in-patient mental health Short Stay unit. The primary focus of the SWII will be centered on discharge planning and placement to ensure seamless patient care and transition back into the community upon discharge. The SWII will assess, plan and implement care of patients and work in consultation with the patient’s treatment team and other community providers to meet the needs /complex needs of the patients following the principles of social determinants of health. The SWII will have ongoing assessment and evaluation of real or potential risks to the patient and family and recommend interventions to mitigate those risks. This position has transitioned to Recovery Alberta effective September 1, 2024. Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Basic Cardiac Support- Health Care Provider (BCLS-HCP) certification required. Demonstrated ability to collaborate with other service providers and to adapt practice to the unique needs of a range of patients, their families and community service providers required. Excellent knowledge and understanding of the Assured Income for the Severely Handicapped (AISH) program. Employment Insurance (EI) program, social service and how to assist patients in accessing these programs as required. Strong mental health assessment and crisis management skills required. Solid knowledge of intervention and treatment modalities relevant to clinical disorders, psychosocial and environmental problems required. Comprehensive knowledge of relevant community resources and legislation required. Experience in discharge planning for patients transitioning back into the community required.

Preferred Qualifications:

Minimum 2 years Social Worker experience providing clinical services to inpatient Addictions and Mental Health clients preferred.
