Your Opportunity:

Working collaboratively and as part of an interdisciplinary team the Social Worker II is involved in the provision of Social Work services to patients and families who receive services from the Alberta Healthy Living Program. Referred clients may require support in their management of a variety of chronic conditions including Cardiac Rehabilitation, Heart Function, Diabetes, Weight Management, Chronic Pain, Stroke recovery, COPD, to name just a few. The Social Worker II provides bio-psychosocial assessment, resource navigation, education, supportive counselling, advocacy, and consultation as part of their day-to-day work. The Social Worker II has an excellent working knowledge of motivational interviewing, Health Change Methodology, and can support clients to identify “self-management” strategies to support their needs. The Social Worker works both on an individual basis with clients and is also comfortable in facilitating groups/ classes for specific client groups. The Social Worker makes appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of patients and families. This position requires the ability to prioritize workload, analyze complex problems and identify viable care plans and solutions to address psychosocial needs. The incumbent demonstrates the ability to work both independently and as a member of an interdisciplinary team. The Social Worker II contributes to non-patient care activities including program planning, evaluation, quality improvement research and staff development, along with other duties as required by the position.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum 2 years of experience working in a multi-disciplinary environment. This position requires a high degree of independence, critical thinking, and problem-solving skills, an ability to prioritize client-centered goals. Knowledge and ability to use computer applications such as Microsoft Office, Outlook and Teams is highly recommended. Previous experience working in the health care system is an asset.

Preferred Qualifications:

Knowledge of Connect care/EPIC or other EMR system. Excellent verbal and written and therapeutic communication skills. Prior knowledge and understanding of chronic illness, chronic pain, grief/loss/trauma, stress management, health promotion, and principles of client self-management are an asset to this role.
