Your Opportunity:

The Neuro Rehabilitation programs, including the Early Supported Discharge team and the Physical Medicine and Rehabilitation clinics, support a population ranging from stroke, brain injury, spinal cord injury, or other neurological injury/disorder. Reporting to the Unit Manager, the Social Worker works within an inter-professional team to support patients who may have complex psychosocial situations including addictions, complicated family dynamics, mental health issues, and other pre-morbid risk factors requiring social work advocacy and intervention. Your services may include psychosocial assessment, resource-based intervention, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion, and public awareness activities. Assessment and intervention are provided in the community, in patient homes, and in the Physical Medicine and Rehabilitation clinics. As such, this position offers the opportunity to be very flexible, adaptive, and innovative in a patient and family centered approach to rehabilitation. The Early Supported Discharge Team is connected to many other stakeholders including allied health and nursing staff, coordinators, and physicians along the continuum of care. The incumbent will be under the general supervision of the Clinical Leader and may be required to take on acting Clinical Leader (CL) responsibilities when the CL is absent. Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Completion of bachelor’s degree in social work (equivalent education or experience may be considered). Active registration with the Alberta College of Social Workers (ACSW). Current Basic Life Support (BLS) certification to be completed annually. Experience working with clients/families within neuro rehabilitation over the last 5 years. Intermediate computer and cell phone skills including the use of Microsoft Office and Connect Care. Ability to perform all duties in accordance with related legislation. Ability to perform all duties in accordance with related legislation. Physically able to perform all assigned tasks: Lifting and transferring patients as required, Pushing and pulling items of approximately 50 kgs, reaching horizontally or above the head, Walking, standing and/or sitting for extended periods, Computer work. Possession of a valid driver’s license, vehicle insurance, and access to a motor vehicle in good repair for work-related travel. Demonstrated strong written/verbal communication and interpersonal skills. Demonstrated ability to communicate and function effectively within a patient and family centered team of health care professionals and support staff. Proven initiative, critical thinking, and problem-solving skills. Demonstrated ability to work independently and be creative. Demonstrated excellent organizational and time management skills.

Preferred Qualifications:

Minimum 3 years of experience in neuro rehabilitation within the last 5 years. • Experience working with clients/families in a community or home setting. Demonstrated continuing education or research in the area of neuro rehabilitation. Experience providing group teaching or education. Participation in or leading quality improvement projects.
