The South Health Campus (SHC) is more than just a “hospital.” It is a campus environment that includes inpatient, outpatient and wellness services, The SHC has four key pillars of service: Collaborative Practice, Wellness, Patient and Family Centered Care, and Innovation. These pillars are guiding the development and delivery of service to all patients and families at SHC. The Allied Health department at SHC provides services to inpatient units and outpatient service areas. This position is part of the Allied Health departments Outpatient Social Work team which provides service to the Neurology, Medicine, Cardiac, Pulmonary and Surgical clinics. This is a unique opportunity to work with interdisciplinary teams serving a broad range of patient populations. Under general supervision from the Allied Health Assistant Manager and in consultation with the SW Clinical Practice Leader, the Social Worker will seek to empower patients to address the social drivers of health that are impacting their chronic illness. This position requires Social Work screening, assessment, and intervention, including counselling for illness adjustment. The successful applicant will support and liaise with patients and their families, colleagues, and internal/external agencies to provide coordinated interdisciplinary care. The applicant will engage in continuous professional development and education and be willing to undertake supervision and development of Social Work students. Effective time management and excellent written and verbal communication skills are essential. The applicant will be skilled at developing collaborative partnerships with community programs to help facilitate access to services for ongoing needs. The caseload for all positions within the Allied Health department may be subject to change based on operational considerations.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 2 years post-degree (Social Work) experience. Demonstrated experience working with patients who have a chronic illness. Demonstrated skills in assessment, intervention, system navigation and case management.
Preferred Qualifications:Experience working within a chronic disease management framework with adult populations in an outpatient setting within the last 5 years. Experience providing education to patients and families about chronic illness management. Experience providing counselling. Experience and knowledge of community & financial resource. Experience working in an adult acute care setting within the last 5 years. Experience facilitating group work. Demonstrated dedication top professional development.
