Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum of five (5) years’ Social Work clinical experience. Minimum of three (3) years’ experience within the last five (5) years in Palliative Care. Minimum of one (1) years’ experience within the last five (5) years Continuing Care, Brain Injury, Mental Health, or other relevant clinical area. LEAP and/or palliative care training. Experience with elderly or vulnerable populations, brain injury, addiction and mental health or developmental disability patients. Able to work independently in different environments (weather conditions) and must be physically capable of carrying approximately 25-30 lbs. Connect Care trained.
Preferred Qualifications:Knowledge of Continuing Care service, including delivery options, assessment/placement criteria, and community supports and agencies. Additional preparation in Psychiatry, Mental Health and/or Palliative/end of life care. Competent and comfortable with MS office suite and virtual meeting venues (TEAMs, ZOOM etc.). PIECES and/or Supportive Pathways trained. Capacity assessment training. Completion of McMaster Case Management training- Level II. Previous experience with providing adult education.
