Your Opportunity:
Reporting to the Manager and leadership of Mental Health Supported Housing, the Social Worker II works as a member of the AMH Supported Housing Program, primarily supporting client transitions - providing intake support, case management and discharge/transition planning. The team is comprised of Social Work, Nursing, Outreach Workers, and Psychiatry. This team functions to provide treatment, rehabilitation, support services, transition planning, and case management functions for clients with severe and persistent mental illness, who require a supported housing environment to live successfully in the community. Team members work both interdependently following a case management model, while sharing responsibilities for duties such as facilitating group therapy programs, medication management, CTO management and coverage for other team members as needed. The Social Worker is an assertive, proactive individual with a strong foundation in the area of addiction/mental health and housing services in the Calgary Zone. The social worker possesses the ability to interact in a wide range of challenging circumstances, using critical thinking and complex problem-solving skills in order to provide quality, client-driven, culturally appropriate, and integrated services that are safe, competent and ethical. Please note: This position is an outreach position and may involve the Social Worker visiting clients in their home and also transporting clients in their personal vehicle. Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Active registration and practice permit with the Alberta College of Social Workers (ACSW). Authorization to Perform Restricted Psychosocial Interventions with the ACSW. Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). 3-5 years adult mental health experience within the past 5 years. Experience working with individuals with a severe and persistent mental illness. Knowledge of the Addiction & Mental Health Care Continuum, including flow and capacity. Knowledge of housing resources and barriers to mental health housing. Knowledge of Mental Health Legislation, especially Community Treatment Orders. Knowledge of AMH philosophies and initiatives – Recovery Model, Trauma Informed Care, Harm Reduction, LOCUS, Collaborative Problem Solving, etc. Experience in conducting psychosocial assessments, treatment planning, and crisis intervention. Self-motivated with proven ability to work both independently and collaboratively within a multidisciplinary team. Demonstrated responsible decision-making through the application of knowledge, critical thinking and sound legal and ethical judgment. 1-2 years of Addiction and Mental Health case management experience. Excellent organizational, problem-solving and time management skills. Experience with MS Office Suite, Outlook, Connect Care and STRATA.
Preferred Qualifications:Experience working in community mental health outreach. Group therapy experience.
