Your Opportunity:

The Social Worker II works within an interdisciplinary care team and will specifically support the timely transition of patients/clients with the goal of reducing Alternate Level of Care (ALC) days in acute care. This position will support the South Zone with the achievement of specific provincial and zone transition/placement targets. The position reports to the Allied Health Manager for Social Work and influences timely client transitions by addressing complex psychosocial, legal and/or financial barriers that contribute to delayed transitions or placement in appropriate community living options. The Social Worker works with the patient and family to provide holistic psychosocial assessment and ensures that patients have access to the necessary financial benefit programs to be able to pay accommodations fees (e.g., Alberta Seniors Benefit, AISH, and federal pensions), and to address other financial issues including the filing of back taxes, asset discovery, banking, and account management, etc. The Social Worker II is responsible for supporting clients/families in the use of legal decision-making mechanisms including Personal Directives, Powers of Attorney, Guardianship and Trusteeship applications and makes referrals as appropriate. The Social Worker assists with navigation of complex family dynamics, including assessment for risk factors such as elder abuse, domestic violence, caregiver burnout, etc. that may impact the timely resolution of these matters. This position provides supportive psychosocial adjustment counselling for patients and families who are navigating an often-challenging life transition to a continuing care living option, including counselling, crisis intervention, conflict resolution, and mediation. This position is based out Chinook Regional hospital but will include support clients in the community.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum 2 years of experience in the health care system. In-depth knowledge of Income Support programs and allowances at a community/provincial/federal level (AISH, Alberta Works, CPP, OAS, ASB) is required.

Preferred Qualifications:

The incumbent will demonstrate a high degree of independence, critical thinking, and problem-solving skills, an ability to prioritize workload. Knowledge and ability to use computer applications such as Microsoft Office, Outlook and Teams is preferred, Experience working in an interdisciplinary team is preferred. Knowledge of relevant legislative Acts and experience with guardianship, trusteeship, personal directives and capacity assessment processes is an asset.
