Social Worker II
Your Opportunity:
An exciting opportunity exists at Sunridge Medical Gallery as a Social Worker II. You will get to work with clients throughout the Calgary zone. As a Social Worker II (SWK II) you report to the Area Manager and the Care Manager of the Seniors Central, COPD and In Home Rehab team in the Integrated Home Care program and provide a range of social work services to clients, families and an interdisciplinary team. The IHC program supports clients to remain independent in the home. This position is part of an exciting new initiative providing In Home Rehabilitation services to clients. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 2 years social work experience within the last 5 years. Experience in community work is preferred. Considerableinitiative and judgement and the ability to work independently in community settings is essential. Basic computer skills.
