At the Glenrose Social Work Department we actively contribute to a respectful and inclusive workplace. Working as part of an interdisciplinary team, the Social Worker II facilitates the psychosocial adjustment of clients/patients/families to the results of accident/injury, medical conditions, or developmental diagnoses. Duties involve providing patient and family centered care including consent, assessment, intervention, follow-up, documentation, education, advocacy, resourcing, consultation, crisis management, and discharge or transition planning within assigned programs/units/clinics. The Social Worker II participates in non-client care activities including program planning, resource development, student supervision, evaluation, quality improvement, research, and staff development. Responsibilities include participation in activities and initiatives aimed at promoting, maintaining, and improving quality of care and safety for patients, families, and caregivers.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Strong interpersonal, communication, and teamwork skills are required. Proficiency in individual and family assessment/intervention. Understanding of ethics and ethical decision-making. Demonstrated familiarity with community resources is required.
Preferred Qualifications:Counseling and mental health experience preferred. Housing and family support work preferred. Healthcare and hospital experience is preferred. Experience in a rehabilitation setting is an asset. Connect Care is an asset.
