If you’re looking to make the difference in the lives of Albertans and have an impact on the healthcare they receive, the position of a North Zone Specialty Teams Social Worker with our team could be for you. As a Specialty Teams Social Worker you will function as a member of an interdisciplinary consultation team in providing palliative and complex care consultations in acute care, continuing care, home and community care settings to provide specialized assessment and intervention on a consultative and limited direct service basis. You will be responsible for working collaboratively with other healthcare professionals, physician consultants, and primary care physicians, along the continuum of care to ensure the provision of person-centered care, that follows evidence-based best practices. This position supports patients in North Zone, with the position location based out of Westlock, Alberta. The position location may be negotiable within the North Zone. This position includes virtual work and face to face patient consultations. Travel is required in the North Zone.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum of five (5) years’ Social Work clinical experience. Minimum of three (3) years’ experience within the last five (5) years in Palliative Care. Minimum of one (1) years’ experience within the last five (5) years Continuing Care, Brain Injury, Mental Health, or other relevant clinical area. LEAP and/or palliative care training. Experience with elderly or vulnerable populations, brain injury, addiction and mental health or developmental disability patients. Possession of a valid driver's licence and access to a motor vehicle in good repair for work-related travel. Able to work independently in different environments (weather conditions) and must be physically capable of carrying approx. 25-30lbs. Connect Care trained.

Preferred Qualifications:

Knowledge of Continuing Care service, including delivery options, assessment/placement criteria, and community supports and agencies. Additional preparation in Psychiatry, Mental Health and/or Palliative/end of life care. Competent and comfortable with MS office suite and virtual meeting venues (TEAMs, ZOOM etc.). PIECES and/or Supportive Pathways trained. Capacity assessment training. Completion of McMaster Case Management training-Level II. Previous experience with providing adult education.
