Working as part of an interdisciplinary team within the Adult Acute portfolio, the SW will provided the opportunity to advocate for those in need of mental health support. We provide patient and family centered care, with all team members working within the CoACT collaborative care model and the Addiction and Mental Health - Edmonton Zone Principles of Care. Reporting to the Unit Manager for unit 4G2, the Social Worker is part of an interdisciplinary team that facilitates the recovery of Addiction & Mental Health patients by assisting with their psychosocial needs while in the Acute Adult Mental Health Program based at the UAH. Duties include assessment, education, treatment, advocacy, resource finding, consultation and discharge planning for patients and their families. The incumbent will also participate in non-patient care activities such as program planning, evaluation, quality improvement, research and staff development, along with other duties as required by the Unit Manager. This position has been identified and is anticipated to transition to Recovery Alberta effective September 1, 2024. Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

The incumbent must have strong communication skills and must be able to work cooperatively with the health care team members, patients, and their families in a fast-paced environment. An ability to incorporate professional judgment when making decisions is required. Initiative and time-management skills are necessary. Working knowledge of CoACT model of care. Working knowledge of trauma informed care. A good knowledge base of housing options and community resources available.

Preferred Qualifications:

Minimum one years’ recent (within the past two years) experience of working in Adult Acute Addictions and Mental Health, experience in Acute Inpatient preferred. Strata pathways housing experience an asset; Familiar with resources in Edmonton Zone; Training and/or knowledge of LOCUS (Level of Care Utilization System); Connect Care trained/access. Master of Social Work is desirable. An ability to work with various computer programs is required, experience with Connect Care is an asset.
