Your Opportunity:

Description:

As a Rehab Practitioner, you will be an integral member of an interdisciplinary team that focuses on supporting a designated client group. You will be responsible for bringing an active and creative approach to welcoming, engaging, screening, assessing and providing support that maintains the uniqueness of each individual within a wellness-oriented, collaborative service delivery model. You will use approved assessment tools while assessing clients at the beginning, during and at time of discharge from the program in order to monitor progress. You will make treatment plan recommendations and initiate the involvement of other professionals and agencies as applicable. You will effectively introduce and explain the treatment plan to the client, their families, caregivers, and advocates using a patient- and family-centred care approach. You will encourage, plan and implement activities for clients as a means to enhance their community integration.

Required Qualifications:

Completion of diploma in a health related field. Active or eligible for registration with applicable college.

Preferred Qualifications:

Knowledge and experience of positive behavior supports and person centered care approaches including developing and implementation of intervention strategies.
