Your Opportunity:
Wetaskiwin Home Care is seeking an experienced Registered Nurse for its Home Care team. Reporting to the Care Manager, Wetaskiwin Home Care in the role of Case Manager, you will be responsible for coordinating and providing quality care to clients in the community. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning. Through the development of the care plan you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates and be on call.
Description:
As a Registered Psychiatric Nurse (RPN), you will provide leadership through a wide variety of nursing services to patients (clients, residents) for the purpose of promoting, maintaining or restoring health, mental health and preventing mental illness, injury and disability. You will teach, counsel and advocate on behalf of patients and take necessary steps to ensure their safety and well-being. You will provide safe, quality, patient- and family-centered care, while reflecting shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Psychiatric Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Completion of an accredited Psychiatric nursing education program. Active or eligible for registration and practice permit with the College of Registered Psychiatric Nurses of Alberta (CRPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
Additional Required Qualifications:Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel. Driver's Abstract to be completed by AHS with no GDL restrictions. Actively integrates Patient and Family Centered Care principles into practice. Excellent communication and interpersonal skills in order to work collaboratively with members of the multidisciplinary team. Has working knowledge of computer skills, Microsoft office. RAI-HC training. Has Connect Care roles for home Care and Connect Care training for Home Care environment. Has experience with Service Authorizations, Palliative care, Medication Assistance program, Placement process. Has wound care, IV and CVAD, and other direct care experience.
Preferred Qualifications:Minimum one (1) years community nursing experience within Canada. Demonstrates critical thinking, conflict resolution and problem-solving skills. Excellent time management and prioritization skills. Competence in relevant technical nursing skills and a willingness to learn new skills as require.
