Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Minimum 3 years nursing experience required. Previous nursing practice in community, geriatrics, mental health or transition services acute care. Knowledgeable with programs PARIS, Strata pathways, Connect Care. Demonstrated conflict resolution, problem-solving, negotiation and facilitation skills. Excellent organizational and time management skills to efficiently organize and prioritize workload. Ability to be self-directed and exercises independent decision making, nursing judgment and critical thinking skills. Position requires significant walking (including stairs and long hallways) of a minimum of 3-5 km daily, repetitive adjustment and exertion including sit/stand/bending throughout shift, repetitive keyboarding and written documentation for a minimum of 6 hours and carrying, lifting and pulling resource materials between 20-25 lbs. Excellent interpersonal and communication skills to effectively work with a multi-disciplinary team of Physicians, Nurses and Allied Health professionals. Ability to monitor financial accountabilities related to care delivery. Excellent written, verbal and computer processing skills to appropriately capture pertinent client information.

Preferred Qualifications:

IHC, ISFL or TS or acute/sub-acute care experience (prefer transition services) or LTC/supportive living experience. RAI trained/experience preferred. Experience working collaboratively with internal and external partners (continuing care and supportive living) to establish transition planning for funded placement and/or discharge back to community. Strong computer skills and use of variety of software programs some of which include PARIS, Strata Health programs, SCM and Connect Care.
