Your Opportunity:

Reporting to the Edmonton Zone Manager, Home Parenteral Therapy (HPT) Program, the incumbent works as part of a multidisciplinary team that is responsible for assessment of patients referred to the HPT program. This designated float position works closely with other staff including care unit staff, home care, physicians, and other allied health practitioners. The incumbent will ensure the development, implementation, and ongoing administration of client care plans and will be required to work out of both University of Alberta Hospital and Royal Alexandra Hospital sites. Provides Assessment By: Identifying changes in the patient’s specific health patterns and attempting to formulate nursing diagnoses/problem statements and relevant patient care outcomes. Determining the patient’s and family’s ability/desire to participate in HPT program, the necessity to involve other members of the health team in ongoing care, and planning for discharge. Planning: Formulating and individualizing patient plan of care and discharge plan based on current nursing knowledge and HPT policies and procedures. Coordinating arrangements required to meet the patient’s discharge needs. Implement care plan: Implementing the nursing care plan and the medical treatment plan as directed/written. Performing independent nursing interventions utilizing the nursing process to assist the patient to manage and/or resolve health problems. Recognizing and reporting unexpected or unusual symptoms, changes in condition or adverse reaction to drugs or treatments to physician. Evaluate Care Plan: Evaluating and documenting the patient’s and family’s response to care and teaching, including input from other nursing personnel. Modifying/revising the patient’s plan of care based on a reassessment of the patient’s condition and progress.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the last five (5) years, you must complete the following free courses: CRNA Medication Management (Chapters 1-4), available at https://connect.nurses.ab.ca/home/learning-and-development/learning-modules/ and the NextGenU Health Assessment Resources Certification, available at https://courses.nextgenu.org/course/view.php?id=271.

Additional Required Qualifications:

Baccalaureate Degree in Nursing required. 5 years of recent adult and pediatric acute care nursing experience required. Home Parenteral Therapy experience, within the past 6 months, required. Experience and proficiency with ambulatory pumps is required. Current and up to date knowledge and experience in intravenous therapy is required. Extensive knowledge/skills of patient care practices (Adult and Pediatric) directly related to IV therapy, and Central Venous Access Devices is required. Basic computer skills and competence with Netcare and Connect Care are required. Demonstrates excellent organizational, decision making, teaching, time management and communication skills, reliability, and personal suitability. Proven ability to work independently as well as positively, effectively, and cooperatively within an interdisciplinary team. Must demonstrate strong problem solving and conflict resolution skills and be able to adapt in a changing work environment. Physical capability of performing all aspects of the role (bending, crouching, lifting). Must be able to withstand long periods of walking and standing as this role requires frequent patient visits to multiple units across the hospital campus. Also must be able to manually operate and program the ambulatory pumps and lift/carry patient medications and equipment up to approximately 20 pounds.

Preferred Qualifications:

Experience with project planning, process improvement and patient safety/quality initiatives an asset. Experience in practice standards/guidelines development and review preferred.
