Reporting to the Home Care Manager, in the role of RN - Case Manager you will be responsible for coordinating and providing quality care to clients in the community. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. Registered Nurse – Case Manager will interact with the team and client/family to coordinate care planning. Through the development of the care plan, you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates and be on call. Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:Excellent interpersonal, communication, and time management skills. Strong ability to work in a collaborative team demonstrating adaptability and flexibility from day to day. Proven critical thinking, decision-making and assessment skills. Physically able to perform all assigned tasks which may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling-pushing, stooping and carrying which may involve visits to client homes that are not ideal. Demonstrated ability to work independently in a team environment. 2-5 years of RN experience in a long-term care/home care/acute care setting in the last 5 years.
Preferred Qualifications:Experience in providing case management preferred. Experience in Home Parenteral Therapy (HPT); wound, skin and ostomy care; client teaching; palliative support an asset. Service Authorization, RAI-HC and McMaster Case Management an asset. For Internal candidates, completion of Required Organizational Learning and up to date CCHSS education.
