Registered Nurse - Chronic Disease Management
Your Opportunity:
Under the direct supervision of the Chronic Disease Management Manager, and CDM Team Lead, this position supports the delivery of standardized evidence-based cardiac rehab and stroke care to patients across the continuum. This position is responsible for monitoring Primary Stroke Centre compliance with regulatory requirements and patient processes. This is accomplished by collaborating with appropriate department managers in facilitating and encouraging use of Canadian Stroke Best Practice Guidelines and following zone and provincial stroke care protocols. This position will assist in providing trans-disciplinary staff stroke orientation, as well as general stroke education that meet the needs of staff and the facility. This position will participate in and guide education of stroke and cardiac rehab patients, as well as provide the conduit for patients to link with Chronic disease management programs and other community support services after discharge. This position maintains communication channels with and supports the non-primary stroke centers that refer into this site.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:Experience in stroke care, cardiology care, emergency department, or neurology National Institute of Health Stroke Scale training certificate, Swallow screening training applicable for site. Clinical experience with emphasis on neurological conditions or stroke care. Experience in teaching adults. Good communication skills with effective team building capability, experience in change management, and quality improvement initiatives. Familiarity with principles of Health Change Methodology and incorporating them into daily practice. Knowledge and skills with computer and software application. This position will be part of Primary Care Alberta effective November 18, 2024. Preferred Qualifications:Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in the community sector will be considered.
