Registered Nurse - Case Manager
Your Opportunity:
As a casual RN Case Manager with the Dementia Care Team, you will provide a wide variety of nursing services to patients while taking necessary steps to ensure their safety and well-being, within a client and family centered and collaborative frameworks. In your role you will apply knowledge and experience of dementia care best practices and utilize nursing processes, critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. Using a client and family centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. This position is based out of the three Dementia Care Team locations and the successful candidate will be assigned to one of the locations depending on the team's need.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:Two or more years’ experience in dementia care or other mental health focus as registered nurse. BCLS-HCP from Heart and Stroke Foundation within the last 12 months. There is also an expectation to be a mobile worker. This role requires travel within the city of Calgary including caring for clients in their homes or in the community. You will be responsible for operating a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work.
Preferred Qualifications:Bachelor’s degree in Nursing. Experience working with acute clients and/or long-term clients. Experience in providing case management services. Canadian Nurses Association (CNA) certification in Gerontology.
