The Bashaw Home Care program serves Bashaw and surrounding rural communities, providing dynamic, client focused care in a setting where priorities shift quickly and no two days are the same. As a Registered Nurse – Case Manager, you will lead and coordinate care within a busy, demanding home care program, working closely with community partners, acute care, and an interdisciplinary team. Strong communication, adaptability, and the ability to manage competing priorities are essential in this rapid turnover role. This position supports the Temporary Type B / Supported Living beds at Bethany Bashaw Meadows, funded by Alberta Health to facilitate timely discharge from acute care for clients awaiting placement. The workload is very fast paced, requiring excellent clinical judgement, organization, critical thinking, and collaboration to ensure safe and efficient transitions of care. Responsibilities include client assessment, care planning, coordination of services and resources, interdisciplinary communication, and evaluation of outcomes in alignment with professional standards and home care guidelines. Excellent communication skills are essential. Bashaw is located less than 100 km from Camrose, near the Battle River Valley, and offers a welcoming, family oriented community with a K 12 school, grocery store, restaurants, and community centre — allowing you to make a meaningful impact while enjoying a quieter lifestyle.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:Ability to participate in an on call rotation based on operational requirements, with the capacity to respond and report to the site or client’s home within 30–45 minutes when called back. Demonstrated experience in case management, preferably within a community or home care setting, with a strong understanding of care coordination and client centered planning. Previous home care experience is essential, along with a solid working knowledge of the Client Care Home Services Standards (CCHSS) for Home and Continuing Care.
Preferred Qualifications:Knowledge of the continuing care sector and the various levels of care is considered an asset. Familiarity with the interRAI Home Care (HC) assessment tool is preferred. Experience in wound care, including strong knowledge of wound care principles and appropriate wound care product selection and use, is an asset. Experience and training in Connect Care are also considered assets.
