Your Opportunity:
The Cardiac Nurse Navigator (CNN) is critical to the effective and efficient provision of cardiac services at the Mazankowski Alberta Heart Institute (MAZ). The CNN triages and manages patients as they enter cardiac care providing support, education, direction and ensuring preparedness for their journey. Additionally, the role ensures optimization of clinical resources and that wait times meet provincial and national performance standards. The CNN manages access to cardiac services for both in and out- patients as well as maintaining patient flow through program beds on a daily basis. The CNN is involved in quality review processes to improve cardiac services in the Edmonton, North and Central zones. The CNN acts as a lead liaison between patients, physicians and cardiac programs and supports planning, developing, delivering, and evaluating services for cardiac patients within the Edmonton, North and Central zones as well as patients who we serve from other provinces.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:3 to 5 years of experience in cardiac sciences within an acute care setting, with experience gained within the last 5 years. Ability to provide coverage for the Cardiac Transition team, offering patient education in cardiology and cardiac surgery as needed. Capable of transporting patients to and from education classes (including pushing/pulling a wheelchair) and providing CPR in emergency situations. Proficient in administrative tasks and computer applications, including MS Office and Connect Care. Strong verbal and written communication skills.
Preferred Qualifications:Bachelor of Science in Nursing (BSN). Proven leadership excellence, with the ability to influence and collaborate effectively with others. Exceptional interpersonal skills and relationship-building abilities; comfortable working within a team environment. Strong conflict resolution, change management, project management, problem-solving, and critical thinking skills. Ability to thrive in a fast-paced environment, managing interruptions and making quick decisions when necessary.
