Your Opportunity:
Integrated Home Care uses a strengths–based Client Centered Case Management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. The Registered Nurse will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. Client care in a Home Care setting uses a collaborative, team based approach while working remotely in a community setting. There is an expectation to be a mobile worker as this role requires travel within Wheatland and Rockyview County including caring for clients in their homes or in the community. Ability to work a combination of Days, Weekends & On-Call. You will be responsible to operate a laptop computer and mobile phone, and manual handling (lifting, lowering, carrying, pushing and pulling) is a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:Two years of recent home care or acute care experience in the last five years. Advanced wound care knowledge and experience required. Must be proficient in computer use as role is heavily dependent on computer use. Knowledge, skill ability and willingness to work to full scope of practice. Ability to use critical thinking skills. Critical inquiry to assess, plan & evaluate.
Preferred Qualifications:Experience working with acute clients and/or long term clients with a focus on chronic disease management. Experience in providing case management services. Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support and client teaching.
