Edmonton Zone Continuing Care provides services through four main program areas: Home Living, Supportive and Facility Living, Transition Services and Palliative and End of Life and Community Programs. The Project Manager, known therein as Complex Care Coordinator (CCC), will work with a high degree of independence and advanced skills to safely support the discharge of residents with complex psych/behavioral/clinical challenges, from acute care. CCC ensures that Enhanced Support and Specialty Units provide safe care for complex residents, stabilizing and then facilitating transition to conventional beds within Type A/B CCH, maximizing flow and resources. CCC also responds to challenging care needs for residents already residing in Type A CCH sites. The successful candidate will lead and develop creative solutions in collaboration with Type A operators to deliver safe care to residents with complex needs. You will support operators to develop successful behavioural support plans by applying your advanced clinical skills and best practice knowledge and implementing boundary setting principles. You will build teams’ capacity to provide safe, sustainable, and quality care to residents with complex needs. CCC has a key role in stakeholder relations and in planning and organizing the work of the portfolio. CCC will also manage process improvement initiatives and align project outcomes with corporate priorities and goals, program needs and as zone issues evolve. CCC will contribute to strategic planning, business case development, system design, change management, and systematic evaluation of programs and services within the Continuing Care portfolio.
Description:

Undergraduate degree in Nursing. Minimum five to seven years in health care. Experience in continuing care (preference in Type A facilities). Advanced understanding of clinical ethics and behavioral health assessment and interventions. Able to demonstrate a clear pattern of professional and personal development. Possesses successful leadership, interpersonal skills and knowledgeable in proven collaborative practice models. Demonstrated ability to facilitate change. Team leadership skills, including demonstrated ability to inspire and build confidence in others. Demonstrated ability to foster relationships and partnerships with internal and external stakeholders. Effective communicator with strong organizational skills, influential skills client/ patient focused and commitment to providing quality services.

Preferred Qualifications:
Experience working in Connect Care. First-hand experience managing complex residents within Type A facilities. Proven critical thinking skills and ability to come up with creative solutions to seemingly impossible situations.
